Yes. Coverage is limited to a maximum of $10,000 for all expenses incurred outside of Canada.

Questions concerning claims should be directed to Beneva Inc.

Contact Beneva Inc. in writing, outlining your reasons for not meeting the deadline. Please note: No claim will be considered later than one year after the date of the accident.

Yes. Claims should be submitted to Beneva Inc. within 30 days of the date of the accident for which the claim is being submitted.

No. If your claim is under $100 or for an ambulance bill only, the Physician's Statement need not be completed.

Claims for medical expenses can be made by completeing the Student Accident Insurance: Non-Dental form in its entirety.  This is a single form comprising 3 pages.

  • Page 1: The Claimant's Section is to be completed by the student/parent or guardian.  The School Declaration is to be completed by the school principal.
  • Page 2: Is to be completed by the student/parent or guardian.
  • Page 3: The Physcian's Statement section is to be completed by the attending physcian, but does not need to be completed for claims $100 or less or for ambulance bill only.

Claims for dental expenses can be made by completing the Student Accident Insurance: Dental form in its entirety.  This is a single form comprising 3 pages.

  • Page 1: The Patient Information section is to be completed by the student/parent or guardian.  The School Declaration is to be completed by the school principal.
  • Page 2: Is to be completed by the student/parent or guardian.
  • Page 3: The Attending Dentist Statement is to be complted by the dentist.

No. The Gold Plan can be purchased at any time during the school year for $14.00, but the premium will not be pro-rated.

Coverage can be purchased by completing the application attached to the SIP Student Accident Brochure and sending it along with a cheque or money order to SIP. There is also a payment link where parents can purchase the coverage on-line.
