How do I make a claim?

Claims for medical expenses can be made by completing the Volunteer Accident Insurance: Non-Dental form in its entirety. This is a single form consisting of 3 pages.

  • Page 1: The claimant's section is to be completed by the volunteer. The School Declaration is to be completed by the school principal.
  • Page 2: Is to be completed by the volunteer.
  • Page 3: The Physician's Statement section is to be completed by the attending physician, but does not need to be completed for claims $100 or less or for ambulance bill only.

Claims for dental expenses can be made by completing the Volunteer Accident Insurance: Dental form in its entirety. This is a single form consisting of 3 pages.

  • Page 1: The patient information section is to be completed by the volunteer. The School Declaration is to be completed by the school principal.
  • Page 2: Is to be completed by the volunteer.
  • Page 3: The attending dentist statement is to be completed by the dentist.
